All mandatory information is collected pursuant to section 26© of the Freedom of Information and Protection of Privacy Act (FOIPPA). Information from this registration is used to measure participation of our society’s programs and for program research and evaluation. We do not share any personal data of our learners with other agencies. This form will be destroyed once the learner leaves our programs. 1. The information in this registration form is, to the best of my knowledge, complete and correct. 2. I understand that personal information from this registration may be used for the purpose of research and evaluation. Any information released will be in a non-identifiable form. 3.
